Post-Hospital & Transitional Care
Coordinated support for suitable patients following hospital discharge, developed with referring healthcare organisations and appropriate clinical partners.
Knightsbridge Healthcare is developing a future Saudi clinical initiative focused on post-hospital, chronic, frailty and palliative care, with the aim of supporting safer transitions between hospital and community-based care.
Our model is currently in development. Clinical services are not yet being provided by Knightsbridge Healthcare in Saudi Arabia and will commence only through the appropriate Saudi legal, clinical and regulatory framework.
Knightsbridge aims to explore how a carefully governed community-care model could complement these priorities by connecting hospital care, professional healthcare support and the patient’s home.
We aim to uphold Saudi Vision 2030 emphasis on integrated care, chronic-care pathways, palliative care, improved access, better patient experience, workforce capability and sustainability.
Proposed service areas subject to Saudi licensing, partner agreements and regulatory approvals.

Planned nursing pathways may include wound care, treatment support, monitoring and other home-based nursing interventions within the scope of applicable Saudi licences.
Person- and family-centred support for people living with serious or life-limiting illness, with particular attention to comfort, communication and continuity of care.
Community-based support for people living with long-term conditions, frailty and complex health needs, with an emphasis on coordinated care and appropriate escalation.
Planned rehabilitation pathways supporting mobility, function and recovery following illness, surgery or hospitalisation.
Future exploration of appropriately licensed home diagnostics, remote follow-up, telehealth and care coordination to support continuity between healthcare settings.

Work with prospective Saudi healthcare partners to understand priority gaps in hospital-to-home and community care.
Develop pathways with Saudi institutions, appropriately licensed providers and international expertise where relevant.
Complete the required corporate, healthcare, professional, data and operational approvals before patient-facing activity begins.
Begin with a limited, clearly governed clinical pathway rather than attempting to launch multiple services simultaneously.
Evaluate safety, patient experience, utilisation, operational viability and partner value before wider expansion.
Connect with Knightsbridge Healthcare to discuss hospital-to-home pathways, prospective partnerships and carefully governed pilot development.
Important: Knightsbridge Healthcare is currently a pre-incorporation healthcare initiative. The clinical services described on this page are proposed future activities and are not currently being offered or provided by Knightsbridge Healthcare in the Kingdom of Saudi Arabia. Any future clinical activity will be subject to establishment of the appropriate legal entity, applicable Saudi healthcare and professional licensing, contractual arrangements and other regulatory approvals. References to proposed collaborations do not imply regulatory approval, accreditation, endorsement or a commitment to deliver services.
